Mission:
In line with our University's mission, the 504 Accommodations Program is in place to provide and/or help students acquire the necessary resources and accommodations needed (as verified through proper documentation) to help them become the most effective professionals and leaders of society.
Purpose:
The ADA 504 Accommodations Counselor's purpose is to serve as a liaison between the student and faculty member to ensure that students with disabilities are afforded equal opportunities to demonstrate their academic skills and potential through reasonable and beneficial accommodations.
Student Eligibility:
1. Students must first meet the admissions requirements of ¶¶Òõ¶ÌÊÓƵ and be registered the semester they request services.
2. Students must self-report to the ADA 504 Accommodations Counselor to fill out an intake form.
3. Students must provide documentation of their disability that is no more than three (3) years old. The documentation should include; how you learn, suggested accommodations and fundamental impacts.Â
4. The documentation verifying your disability should be from a psychologist, psychometrist, medical doctor, etc. (a qualified professional).
